چکیده

Objective: To find out various complications after total laryngectomy, with reference to their presentation, diagnosis, and management. Design: It was a prospective descriptive study. Setting: Otolaryngology, Head and Neck Surgery department, Pakistan Institute of Medical Sciences Islamabad, from 1st July 1999 to 31st June 2001. Patients and Methods: 25 patients undergoing total laryngectomy for proven carcinoma larynx (Ca larynx) were included in this study. Patients were reviewed after surgery and any complication that occurred was recorded. The presentation, diagnosis, and management of these complications are discussed, along with voice rehabilitation after total laryngectomy. Results: There were 23 males and 2 females. Age of patients ranged from 40 – 65 years. Complications included wound infection (06), pharyngocutaneous fistula (03), pharyngeal stenosis (03), and nodal metastasis (02). Conclusions: Wound infection and pharyngocutaneous fistula are most common complications after total laryngectomy. Preoperative radiotherapy and low postoperative haemoglobin level are important risk factors for development of pharyngocutaneous fistula in total laryngectomy patients. Postoperative radiotherapy to the neck reduces the risk of nodal metastasis after total laryngectomy.

روش جدید ترمیم زخم پس از لارنژکتومی توتال

Background: Total laryngectomy is the mainstay of treatment for laryngeal cancer. Fistula is one of the most common complications after total laryngectomy. In patients without risk factors such as prior radiotherapy, diabetes mellitus or chronic renal disease, the incidence of pharyngocutaneous fistula is related to wound healing and duration of operation. We have developed a new method that is...

Influential factors, complications and survival rate of primary and salvage total laryngectomy for advanced laryngeal cancer.

This is a retrospective review of patients with advanced malignant neoplasms of the larynx treated with total laryngectomy. 387 total laryngectomies for advanced squamous cell carcinoma of larynx performed in the period between 1995 and 2007 were analyzed. Primary total laryngectomy (PRT) was performed in 316 patients, while initial radiotherapy radiotherapy (60-70 Gy) and concomitant chemother...

Survival after Salvage Total Laryngectomy: The Influence of Previous Treatment

Purpose: To compare the complications and survival rate after different treatment modalities of advanced laryngopharyngeal cancer. Methods: Retrospective study included 619 advanced laryngopharyngeal carcinoma, treated with either primary total laryngectomy (PTL), or salvage (STL) after partial laryngectomy, radio, chemoradiotherapy. Complications and survival rate were documented. Results: Fiv...

Open Maximal Mucosa-Sparing Functional Total Laryngectomy

BACKGROUND Total laryngectomy after (chemo)radiotherapy is associated with a high incidence of fistula and therefore flaps are advocated. The description of a transoral robotic total laryngectomy prompted us to develop similar minimally invasive open approaches for functional total laryngectomy. METHODS A retrospective study of consecutive unselected patients with a dysfunctional larynx after...

Primary and salvage total laryngectomy. Influential factors, complications, and survival.

PURPOSE In this retrospective study we analysed patients with advanced squamous cell carcinoma of the larynx and hypopharynx treated with primary total laryngectomy (PTL) between 1990 and 2007. METHODS The patients were treated by classical PTL, radiotherapy 60-70 Gy, concomitant radio and chemotherapy (cisplatin and 5-fluorouracil) or salvage total laryngectomy (STL). They were followed up f...
